Provides inpatient consult care and home visits for frail elderly patients, with deep expertise in physical rehabilitation, nursing, and information management approaches relating to the care of older adults.

 

Laura Frain, M.D., M.P.H. – Director of Ambulatory

Leads ongoing collaborations with primary care to develop and implement new models of co-managing outpatient geriatric patients.

 

Houman Javedan, M.D. – Clinical Director

Innovates and implements co-management programs across key specialties within BWH, using the current infrastructure of tertiary services, and adding new care pathways with a dedicated team of geriatricians.

 

Samir Tulebaev, M.D. – Geriatrician

Develops geriatric co-management service with the Trauma, Burn, and Surgical Critical Care Service to clinically reduce ICU readmissions, in-hospital mortality, and 30-day mortality among geriatric trauma patients.
